  5. A statement that the complaint has a genuine belief that the material’s use is unauthorized;
  6. A statement confirming the accuracy of the information provided and the authority of the complaining party to act on behalf of the copyright owner.

Counter Notification – Restoration of Material

  3. A statement, under penalty of perjury, that the removal was a mistake;
  4. Your name, address, phone number, and consent to court jurisdiction.
